Tips on Filing CRIS Reports

There are three types of reports entered through the CRIS system for Hatch research projects. Through these three sets of forms we are able to capture the objectives, progress, and inputs required for a given research project...read more.

1. Form AD 416/417/2008 :
   Click here for instructions on the CRIS forms used to start a new Hatch project or revise and existing one.
2. Form AD 421:
   Click here for instructions on the CRIS form used to enter progress reports (or termiantion reports for Hatch projects that have ended) that are required at the end of the fiscal year (entered in October).
3. Form AD 419:
   This form is used to account for the funds and staff support used on a given research project. This report is completed at the end of the fiscal year by the department business office.
